Super Shots is providing a FREE basic physical for middle school and high school students for school and sports for ALLEN COUNTY residents. Reminder - BLHS requires current athletes to have a new physical dated AFTER April 1, 2026, and must be uploaded by July 1st to be eligible to participate in any summer workouts and/or practices. INCOMING FRESHMEN athletes must have a physical on file immediately. Physical Form is attached to print.
